Credit card registration forms ( found in the Fall Bulletin or here in Acrobat and Rich Text Format) are to be filled
and sent by mail or fax to:

To cancel your registration, please contact Steve Canton in writing before May 1, 2000.
A $50 charge on your registration fee will be levied on refunds prior to this date. No
refunds will be issued after May 1, 1999. We will try to confirm receipt of your
registration by e-mail, if you include a valid e-mail address. Otherwise, your
registration receipt will be in your registration packet at the meeting.
